To complete my registration in the Donate Life Maryland Donor Registry as an organ eye and tissue donor, I accept these terms and conditions and state as follows.

I affirm that I am the applicant described on this application and that the information I have entered in the Registry is true and correct to the best of my knowledge.

I affirm that I want to be an organ, eye and tissue donor and that this registration records my decision to make an anatomical gift by donating organs, eyes and tissues for the purposes of transplantation, education, research and therapy. The Maryland Uniform Anatomical Gift Act provides that this registration is a legally effective anatomical gift; by accepting these terms and conditions, I understand that my registration is a document of gift and that making this affirmation electronically has the same effect as signing a written statement. I further understand that I can access the Registry at any time, using the username and password that I created as part of my registration, to change or revoke my registration, to update my information and to give more detailed instructions about my gift.

I understand that a document of gift, not revoked by the Donor before death, cannot be amended or revoked by any other person without the Donor’s express consent , and that that the gift is effective without the authorization or consent of any other person.

I understand that anatomical gifts take effect at death and that the relevant procurement agencies are required to determine whether the gift is medically suitable and safe for its intended purposes. I understand that hospitals notify the organ procurement agency of all deaths or imminent deaths in the hospital. After receiving notice, the procurement agency will carry out its responsibility and help give effect to my donation decision by conducting the tests and examinations needed to determine medical suitability. These tests and examinations will include, among other things, blood tests, and review of your medical records.
